Abstract
A 74-year-old heart and kidney transplant recipient on long-term immunosuppressive drugs developed a rapidly enlarging well-differentiated cutaneous squamous cell carcinoma (cSCC) of the scalp. Delays related to multidisciplinary reassessment and intercurrent sepsis allowed for substantial tumour progression, ultimately precluding surgery and necessitating palliative radiotherapy. This case highlights that even well-differentiated cSCC may progress rapidly in patients who are immunosuppressed, underscoring the importance of timely treatment whenever feasible.
